  Position: RN Care Navigator, Continuum Care Mgr Inpatient----*$10K Sign-On Bonus*
  Location: Evanston--
  
• Full Time/Part Time: Full-Time (40hrs/wk)
  Hours: 8:00AM - 5:00PM, on call and some weekend rotation may be required as the role evolves
  
• Required Travel: Travel to other locations required

  What you will need:
  
  • 
    
  • License: Current professional Nursing Licensure in the state of IL required
    
  • Education: Bachelor's degree required.--Bachelor's degree in Nursing from an--NLN accredited school of nursing required; BSN strongly preferred
    Certification: Basic Life Support for Healthcare Providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross). Clinical certification, such as case management certification, is beneficial
    
  • Experience: 2+ years of clinical nursing experience preferred. Minimum of 2 years of utilization review, discharge planning, case management or disease management preferred. Nursing experience in home services, ambulatory services working with high-risk patients beneficial. Experience with Microsoft Office Suite. Experience with Electronic Medical Record (EMR) platform preferred
    Skills: Able to communicate and work collaboratively with a range of stakeholders and team members; Knowledge of community resources.

    What you will do:
    
    
    Responsible for the utilization management, case management, and care coordination management of his/her population of patients from the time of diagnosis (could be pre-admission or in the ED) to 90 days' post-discharge.--
    Helping patients understand their diagnosis, treatment options, and available resources and ensure that they are connected with the optimal resources across the continuum of care. This role will coordinate discharge planning by facilitating smooth transitions of care while ensuring quality cost-effective patient outcomes.
    Serves as a liaison between their--patient population and all other providers.
    Will be responsible for key metrics of success, which include improving the overall cost of care, length of stay optimization, reduction in excess days, reduction in SNF utilization and improvement in SNF care transitions, reduction in 30-day readmission rate and ED utilization.--
