What you will need:
License: Current professional Nursing Licensure in the State of IL
required
Education: Bachelor's Degree required. Graduate of a NLN accredited
school of nursing required; BSN strongly preferred
Certification: BLS/CPR certification for the Healthcare Provider
required
Experience: Minimum of 2 years clinical nursing experience
preferred
Skills: Knowledge of community resources. Experience with Microsoft
Office Suite. Experience with Electronic Medical Record (EMR)
platform preferred. Proven leadership skills. Ability to work
independently, setting priorities to coordinate care plan
efficiently. Strong interpersonal and oral communication skills, as
well as strong computer and data entry skills.

The Transition of Care RN is responsible for providing telephonic
outreach to patients post hospitalization. Daily functions include,
but are not limited to contacting patients by phone within 48 hours
of discharge with focus on reviewing hospital discharge
instructions, medication adherence, management of patient's quality
of life and functionality, rectifying gaps in care, and assisting
in scheduling follow up visits with their physician. The Transition
of Care-RN will function as an integral member of a collaborative
and interdisciplinary team, using validation, implementation,
coordination, monitoring, and evaluation skills.

What you will do:
Contact patients by phone within 48 hours of discharge to review
hospital discharge instructions, medication adherence, management
of patient's quality of life and functionality, rectify gaps in
care, and assist in scheduling follow up visits with their
physician.
Collaborate with the physician office and members of the care team
to resolve any issues identified during the call.
Document the contact in the medical record in compliance with
documentation requirements.
