Description: Prescribes, coordinates, and delivers care through the
nursing process. Maintains the standards of professional nursing.
Supervises CNA and coordinates care with other healthcare workers
as assigned. Provides comprehensive initial assessments to all
clients and, on subsequent visits, continually assesses physical,
psychosocial, environmental, and community resources available.
Utilizes the principles of growth and development over the
lifespan. Initiates Plan of Care, re-evaluates the need for care,
and modifies Plan of Care as ordered or needed. Establishes
discharge plans with the patient, physician, other disciplines, and
caregivers. Completes OASIS data. Provides patient care using
current federal, state, and agency regulations. Assesses
educational needs of patients, families, and caregivers, and
implements appropriate teaching strategies. Completes documentation
according to regulatory guidelines. Promotes a clean and safe
environment of care, utilizing error prevention habits. Provides
the highest standard of privacy and confidentiality in matters
involving patients and coworkers by abiding by the Standards of
