Job Description Job Description This is a Physical Therapist opportunity in the home health industry with a very reputable and established hospice and home health company. The ideal candidate is a skilled professional that is passionate about delivering quality care and making a real difference. Location is in the greater Lansing area M.I. Responsibilities Assess each patient and develop an appropriate treatment plan with measurable goals and time frames Assessing patient mobility, developing individualized treatment plans, and providing in-home therapy to improve function, manage pain, and prevent falls. Implement therapeutic exercises, use manual therapies, and educate patients and their families on exercises and equipment to promote independence and a higher quality of life within the home environment Prepare and submit clinical and progress summaries based on identified and attained goals Recent graduates with a state license will be considered. Qualifications Bachelor's, Master's or Doctor's degree of an accredited physical therapy curriculum Certification in Physical Therapy and valid state license to practice Must have a valid CA Drivers License Healthcare Benefits, Retirement Plan, and PTO are offered with this position.
