Physical Therapist Location: Janesville, WI Agency: Ambition 24Hours Pay: $1,246 per week Shift Information: Rotating - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Certified Occupational Therapy Assistant (COTA) Skilled Nursing/Rehab Janesville, WI Role overview A Certified Occupational Therapy Assistant is needed for a full time day shift contract at a skilled nursing/post acute rehab facility in Janesville, WI, working under the direction of a licensed OT with adult and geriatric residents. This is a 13 week new opening focused on ADL independence, functional mobility, and safety in a SNF environment. Assignment details Location: Skilled nursing/post acute rehab facility, Janesville, WI 53545 Setting: Skilled Nursing Facility (SNF) / long term care and post acute rehab Position type: Contract, full-time, non permanent Contract length: 13 weeks (02/23/26 05/23/26) Schedule: 8:00 AM 4:30 PM, 30 minute unpaid meal break Days: Day shift with scheduling set by facility (Mon Sun coverage as needed) Guaranteed hours: 35 hours per week Gross Blended Pay rate: $33- $40 p/h Order reason: New opening Key responsibilities Implement occupational therapy treatment plans under the supervision of a licensed OT, focusing on ADLs, self care, and functional activities. Lead therapeutic activities to improve upper extremity function, balance, coordination, and functional mobility. Educate patients and caregivers on use of adaptive equipment, compensatory strategies, and home exercise programs. Observe and document patient status and progress, and communicate timely updates to the supervising OT. Complete accurate, timely EMR documentation for daily treatments and progress notes. Required qualifications and skills Active Wisconsin COTA license (or eligibility prior to start). Graduate of an accredited Occupational Therapy Assistant program. Current BLS/CPR for healthcare providers. Recent SNF, LTC, or post acute rehab experience preferred (travel or perm). Strong communication, teamwork, and time management skills
