Job Description Permanent Medical Director – Geriatric Medicine
Position Type : Permanent Specialty : Physician – Geriatric
Medicine Location : Indiana. Shift : Full time, day shift Position
Details: Patient Population: Frail/Elderly (PACE participants 55)
Setting: PACE (Program of All-Inclusive Care for the Elderly)
outpatient/center-based care Responsibilities: Provide medical
direction and supervision of participant care, oversee clinical
outcomes and QAPI, supervise PCPs and APPs, conduct H&Ps and
reassessments, develop treatment plans, lead interdisciplinary team
collaboration, participate in intake/assessments and family
conferences Required Procedures: Physical examinations, chronic
disease management, ordering/evaluating labs and diagnostics EMR
System: Support Staff: Interdisciplinary team including PCPs, APPs,
nurses, therapists, social workers Days Per Month: Full time
On-Call/Weekends: No call, No weekends Submission Requirements:
Board Certified in Internal Medicine or Family Medicine Board
Certified or Fellowship/CAQ in Geriatric Medicine Active Indiana
license (or willing to obtain) NPDB Self Query within 30 days
Minimum 1 year experience with frail/elderly population Supervisory
experience DEA, CDS, BLS (by start) Join us today!
