Occupational Therapist Location: Rockford, IL Agency: Bluebix Health Inc Pay: $45 to $50 per hour Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/31/2026 About the Position Job Summary: The Occupational Therapist (OT) provides home health services to patients, focusing on improving daily living skills, mobility, and overall independence. The role involves evaluating patient needs and implementing individualized therapy plans in a home-based setting. Job Description: Evaluate patients’ physical and functional abilities in a home health setting Develop and implement individualized treatment plans Assist patients in improving daily living and motor skills Educate patients and caregivers on therapy techniques and safety Monitor patient progress and adjust treatment plans as needed Maintain accurate documentation and reports Travel between patient homes for scheduled visits Requirements Requirements: Active Occupational Therapist license in the state of Illinois (required) BLS certification (required) Minimum 1 year of experience as an Occupational Therapist Prior home health experience required Strong patient care and communication skills Benefits Benefits and Compensation: Competitive travel pay package (negotiable) Weekly guaranteed hours Overtime applicable after 40 hours per week and as per policy Opportunity for contract extension
