*Responsibilities:*

* The Care Navigator position has two components: ambulatory and inpatient.
* Ambulatory: The Care Navigator will perform telephonic work. Telephonic work is done on-site at their assigned hospital. Typically, Care Navigators also travel to physician offices and patient homes but community visits are currently paused. Once they do go back into the community, Navigators will be provided a laptop and do some degree of remote work.
* Inpatient: Will be within the hospital. Care Navigator will perform the following duties: utilization management, case management, and care coordination of their patients. Connecting with patients at the time of diagnosis and following them throughout their hospital stay and 90 days post-discharge. Guiding the patient through the health system from diagnosis, testing, treatment, and follow-up care to assist patients with navigating care. Coordinating discharge planning. Ensured quality cost-effective patient outcomes. Cost-effective patient outcomes are measured on these metrics: overall cost of care, length of stay optimization, reduction in SNF utilization and improvement in SNF care transitions, reduction in 30-day re-admission rate and ED utilization.
* The Care Navigator will rotate between ambulatory and inpatient care every 6 weeks.

*Qualifications:*

* *Education: *Bachelor's Degree required.
* *Experience: *2 years of acute care experience required (may be a combination of acute/hospital care, LTC/SNF or LTACH)
* *Certification: *BLS or CPR/AED
* *License: *IL RN License required

*Schedule: *Monday-Friday, 8:30 am to 5:00 pm. Every third weekend is required and one major summer holiday/one major winter holiday.

*Pay Scale: *Starts at $78,800.
